Terms and Conditions

As Legal Guardian for the above child, I waive and release all rights and claims
for damages that I may Accrue against Glide Rec and any and all sponsors of
activities, their successors, coaches and board members and assigns for any
and all injuries suffered by me or my child participating in all Glide Rec events. I understand the risk of bodily injury associated with, and participating in all activities and assume all such risk by requesting entry. Glide Rec provides both accident and liability insurance coverage for all participants.

As parent or legal guardian I hereby give my consent for any emergency medical
treatment as approved by his/her coach or other adult escort in case of illness or injury while participating in Glide Rec activities. I understand that this is to prevent undue delay and assure prompt treatment and only that licensed
medical personnel will be engaged for such an emergency. I hereby understand that Glide Rec strictly enforces a family environment. This includes positive behavior between all participants, coaches, officials and observers. This also includes no smoking and/or alcohol on or near Glide Rec playing facilities.
