Dale Bonnell Memorial Scholarship

Applicants must have participated on a Glide Rec Youth Sports Team
This scholarship is renewable for a period of three consecutive years and may be deferred for up to two years.

Chosen applicants will be scheduled for a personal interview with  the Glide Rec Board of Directors.
Applicants will be contacted by the first week of May.

Part 1 - Personal Information
A. Name of Applicant
B. Mailing Address
C. Parent/Guardian Name(s) and Mailing Address (if different)

Part 2 - Scholastic and Extracurricular Record
A. Include an official High School transcript
B. Glide Rec Activities: List all Glide Rec teams/sports you participated in.  Please be as specific as possible.
C. Statement describing Glide Rec experiences as they relate to you.  Be specific; a coach that had a particular influence or how participating on a team has affected you.
D. School Activities: List school activities such as membership in school organizations, participation on school athletic teams, music, drama, student government, etc.
E. Non-school Activities: List all activities not related to the school, such as civic or community activities; 4-H, Boy/Girl Scouts, youth groups, etc.
F. Honors and Awards: List all awards of achievements earned in and outside of school.  Provide the date and a description of the award.

Part 3 - Educational Plans
Which college/universities/post-secondary schools have you applied/are planning to apply to?  Have you been accepted?
Part 4 - Personal References
Attach three letters of reference.  Only one may be from a high school teacher.
Part 5 - Personal Statement
Scholarship Committees are particularly interested in your ability to organize your thoughts and express yourself.  Write a statement of 300 to 400 words in which you outline your future plans, chosen field of study, reasons for this choice and personal goals you have.
